Required Tech/Experience:
NOTE: client has not established a rigid hard-stop checklist and wants to see what the market produces. Use the below as the primary baseline, but do not over-screen strong Tier 2/Tier 3 candidates.
1. Strong hands-on software engineering/application development experience, ideally including SMART on FHIR application development, FHIR R4/US Core, modern frontend development, backend/API development, and EHR integration. The strongest candidates will have personally shipped an app that launches inside an EHR.
2. Strong understanding of secure healthcare application development, including OAuth 2.0 / OpenID Connect, PKCE, JWT/token lifecycle, PHI handling, HIPAA-compliant development/audit logging, and deployment to a HIPAA-eligible cloud environment. Provider healthcare/EHR experience is strongly desired.
Preferred Tech/Experience (Not required, but moves the candidate to the top of the list):
1. Epic third-party development/integration experience (open.epic, Vendor Services / Connection Hub / Showroom / former App Orchard, Hyperspace embedding, Vendor Test Environment onboarding) and/or demonstrated CDS Hooks development.
2. OMOP and source-to-OMOP mapping; CQL/eCQM or computable clinical logic; CI/CD and automated testing (GitHub Actions, Postman, Playwright); other EHR integration experience (Oracle Health/Cerner, Athenahealth, eClinicalWorks, Meditech); clinical terminology knowledge (LOINC, SNOMED CT, RxNorm, ICD-10).
Specific Outcome This Position Will Produce (Project Info):
Build and/or enhance a secure SMART on FHIR clinical decision support application that can launch within an EHR, exchange clinical data through FHIR R4/US Core APIs, support appropriate authentication/token flows, and operate in a HIPAA-compliant cloud environment. The consultant will help translate clinical/project requirements into working application functionality and support integration, testing, deployment, and delivery of the solution.
Typical Day to Day (Tasks & Responsibilities):
• Develop frontend and backend components for a SMART on FHIR clinical decision support application.
• Integrate with EHR/FHIR APIs and work with FHIR R4 resources such as Patient, Condition, Observation, MedicationRequest, DiagnosticReport, Immunization, Procedure, and AllergyIntolerance.
• Implement and troubleshoot SMART App Launch, OAuth 2.0 / OIDC, PKCE, scopes, launch context, JWTs, token exchange, and token lifecycle.
• Build secure workflows for PHI handling, audit logging, and HIPAA-compliant application behavior.
• Collaborate with clinical, data, product, engineering, and other project resources to translate requirements into working functionality.
• Support testing, CI/CD, cloud deployment, and issue resolution.
• For candidates with the experience: build/support CDS Hooks services and Epic third-party integration/onboarding workflows.
Years’ Experience/Degree Requirements/Certification:.
No specific years, degree, or certification requirement has been provided. Target senior-level software engineers with meaningful hands-on application development experience and demonstrable ownership of relevant healthcare/FHIR/EHR work. HL7 FHIR proficiency certification, CPHIMS, or a clinical informatics certification are differentiators, not requirements.
Qualifications:
• Hands-on SMART on FHIR app development; has shipped apps that launch inside an EHR (EHR launch and/or standalone), including SMART App Launch, OAuth 2.0 / OpenID Connect, PKCE, scopes, and launch context (patient / encounter / user).
• Working experience with FHIR R4 resources (Patient, Condition, Observation, MedicationRequest, DiagnosticReport, Immunization, Procedure, AllergyIntolerance) and US Core profiles.
• Proficiency in a modern frontend framework.
• Experience with a backend language for the CDS service and token exchange.
• Solid auth/security engineering: OAuth 2.0 flows (authorization code + PKCE and client-credentials / SMART Backend Services), JWT, token lifecycle, and secure handling of PHI.
• Experience building HIPAA-compliant applications, including audit logging.
• Experience deploying to a HIPAA-eligible cloud environment (AWS preferred; Azure/GCP acceptable).
• Working familiarity with clinical terminologies—LOINC, SNOMED CT, RxNorm, ICD-10. Deep terminology engineering is preferred rather than required.
Strongly Preferred:
• CDS Hooks: demonstrated experience building CDS services (hooks, cards, deep-linking into a SMART app).
• Epic third-party developer experience—open.epic sandbox, Vendor Services / Connection Hub / Showroom (formerly App Orchard), Hyperspace iframe embedding, and per-site Vendor Test Environment onboarding.
• OMOP common data model and source-to-OMOP mapping.
• CQL (Clinical Quality Language) or other experience translating clinical guidelines/quality measures into computable logic; eCQM experience a plus.
• Automated testing and CI/CD (GitHub Actions, Postman, Playwright).
• Other-EHR integration experience (Athenahealth, Oracle Health/Cerner, eClinicalWorks, or Meditech).
Nice-to-Have / Candidate Differentiators:
• HL7 FHIR proficiency certification, CPHIMS, or a clinical informatics certification.
• Diabetes or chronic-disease clinical domain exposure.
• Bulk FHIR / Flat FHIR for population-level or registry data.
• Prior work in a registry, research, or IRB-governed data context.
• Comfort with data versioning, traceability, and auditability for reproducible reporting.
